The best high-frequency cutting current is pure sine wave without modulation. The cutting is then smooth and the thermal effect is small, thanks to which hemostasis occurs only to a small extent. Because the effects of this wave can be precisely controlled, it can be safely used for cutting bones. In addition, since good coagulation during cutting is one of the basic advantages of electrosurgery, a certain level of modulation is also desirable.Monopolar coagulation is hemostasis of small blood vessels by high-frequency current flowing in the active electrode. After reducing the current density and applying a wide-surface electrode to spread energy over a larger area, the effect is to dry the surface cells without penetration, which results in coagulation. Coagulated surface cells then form an insulating coating, protecting against too deep heat penetration during subsequent applications. The current used for coagulation is usually modulated. The degree of modulation depends on the smoothness of the cut, the degree of hemostasis and the likelihood of tissue damage. Greater current modulation promotes less precise cutting and is associated with some risk of damage to deeper tissues, but also provides more effective coagulation.Programs for diathermy:CUT (cutting)The best current for cutting with an electrosurgical knife is a pure sine wave without modulation, i.e. with a 100% duty cycle.
